چکیده

Ventricular tachycardia is a life-threatening condition. ECG artifact caused by the 4to 6-Hz rest tremor of Parkinson disease (PD) can mimic a wide complex ventricular tachycardia (figure). Misinterpretation of ECG artifact can have grave consequences, including cardiac catheterization, administration of antiarrhythmics, and placement of automated implantable cardioverter-defibrillators.1 Tremor artifact is most prominent in limb leads and represents the action potentials of striated muscle. Artifact in leads I, III, and aVL in this patient suggests left arm rest tremor. Holding the limb or placing electrodes on the chest can minimize this artifact.2 Neurologists should be aware of this ECG artifact to prevent unnecessary cardiac interventions.

Parkinson's tremor mimicking ventricular tachycardia.

We report the case of an elderly lady with Parkinson's tremor whose electrocardiographic (ECG) appearance simulated ventricular tachycardia. As a result she underwent unnecessary clinical intervention. We also highlight the difficulty in correctly identifying a tremor-induced ECG artefact with regard to Parkinson's disease, due to the diverse range of possible clinical scenarios presented by pa...

Tremor-induced ECG artifact mimicking ventricular tachycardia.

Ventricular Tachycardia To the Editor: The ECG from the article by Srikureja et al1 that adorns the cover of the September 12, 2000, issue of Circulation is a striking example of a tremor-induced artifact mimicking a serious arrhythmia. On this occasion, the apparent arrhythmia was ventricular tachycardia and led to the patient’s admission to the coronary care unit, although, in my experience, ...

Teaching Video NeuroImages: Re-emergent jaw tremor in Parkinson disease.

A 77-year-old woman who was diagnosed with Parkinson disease at 51 years of age presented with a 4-year history of intermittent jaw tremor. This was present at rest and declined in amplitude during voluntary movements. Tremor reappeared after the patient bit a tongue depressor continuously for 10 seconds (video on the Neurology® Web site at Neurology.org). “Re-emergent tremor” is a postural han...

Teaching NeuroImages: pseudo-abnormal DaTscan findings in meningioma-induced parkinsonism.

A 71-year-old man presented with a 6-month history of rest tremor and slowness in his left hand. Apart frommild left parkinsonism, neurologic examination was unremarkable. Because response to L-dopa, up to 600 mg/d, was lacking, [I]FP-CIT (I-N-v-fluoropropyl-2bcarbomethoxy-3b-[4-iodophenyl]nortropane) SPECT was prescribed with unexpected results: transverse and coronal slices showed the right s...
